Company ID’s worker killed in casting accident

HUBBARD TOWNSHIP — Ellwood Engineered Castings has identified the employee killed when a malfunction caused molten steel to be poured over him.

The company Tuesday said Jawaylan Patterson, a team member of eight years, was working the overnight shift when he sustained fatal injuries Saturday morning at the facility during an “iron casting process,” the company stated in a news release.

“We continue to extend our deepest condolences to Jawaylan’s family and have offered them our full support,” Pat Callihan, president of Ellwood Engineered Castings, said. “After the incident, OSHA was immediately notified, and we are cooperating fully with their investigation. Our safety team is working to fully research the cause of the incident. We are committed to a full and thorough investigation by our safety team, OSHA and other professionals. We are committed to providing our team a safe workplace and to making sure this kind of tragedy never happens again.”

The company further stated, “Our hearts are broken for the family, friends and coworkers of Jawaylan and we are working to provide them with the support they need at this time.”

A spokesman for the Occupational Safety and Health Administration confirmed Tuesday compliance officers with the agency were onsite at the Hubbard Masury Road plant Saturday “and continue to conduct interviews with employees, the employer and any potential witnesses.”

“No further information will be available until OSHA completes their investigation, which, by law, they have six months to complete,” Scott Allen, regional director for public affairs and media relations for the U.S. Department of Labor, stated in an email.

According to a police report, a witness told police a man, identified in the report by only his nickname, “Jay,” was in a pit when molten steel was being poured into a casting. There was a malfunction with the casting, which caused the material to spill into the pit where the man still was, the report states.

The person operating the crane yelled for people to move and started to move the ladle containing the molten steel away from the workers, but the man was unable to get away in time and the material poured onto him and filled the entire floor, the report states.

The man was dead when police arrived and employees were watering down the area to recover his remains, the report states.

The incident happened just before 5 a.m., the report states.
